The New Hampshire Stroke Collaborative continues to lay the groundwork for a comprehensive stroke system of care in the Granite State. The NH Stroke Collaborative is an interdisciplinary team composed of healthcare, EMS, and rehabilitation providers, as well as public health agencies and advocates, working together to advocate for an improved stroke system of care, from prevention to acute care and recovery for stroke patients. The group is developing stroke identification and transport protocols for EMS providers, establishing a designation of Primary Stroke Centers and linking resources in communities to stroke patients for the best recovery outcomes. The Collaborative hosts its annual workshop in May, Dissolving Barriers to Stroke Care in NH, a forum to identify gaps and create linkages in the continuum of stroke care.
