NH Stroke Collaborative recipient of Community Conscience Award

The NorthEast Cerebrovascular Consortium (NECC) is presenting its prestigious Community Conscience Award to the New Hampshire Stroke Collaborative at its Annual NECC Summit October 2017.  The NH Stroke Collaborative is an interdisciplinary team composed of healthcare, EMS, and rehabilitation providers, as well as public health agencies and advocates, working together to advocate for an improved stroke system of care, from prevention to acute care and recovery for stroke patients, since its inception in 2002.  In May 2017, the Collaborative released an extensive plan designed to provide stroke care providers, public health officials and health policy experts with a guide for integrating public health and stroke prevention with acute care systems and secondary prevention and rehabilitation.  The initial strategy for NH is developing stroke identification and transport protocols for EMS providers, the establishing designation of Primary Stroke Centers and linking resources in communities to stroke patients for the best recovery outcomes.
